Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team at Duke Health. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and services in a home health setting.
Key Responsibilities- Assess and Plan Care: Conduct comprehensive assessments of patients' physical and emotional needs, develop individualized care plans, and implement interventions to achieve optimal patient outcomes.
- Provide Direct Care: Deliver high-quality patient care, including administering medications, performing treatments, and providing education to patients and their families.
- Collaborate with Interdisciplinary Team: Work closely with physicians, therapists, and other healthcare professionals to ensure comprehensive and coordinated care.
- Document Patient Information: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes.
- Communicate Effectively: Develop and maintain effective relationships with patients, families, and healthcare team members through clear and respectful communication.
- Education and Experience: Master's degree in Nursing or related field, with a minimum of 1-2 years of experience in home health or a related field.
- Licensure and Certification: Current RN licensure in NC or compact RN licensure from participating state, with current State of North Carolina Cardio-Pulmonary Resuscitation Certificate (CPR) (American Heart Association only).
- Skills and Abilities: Strong assessment and critical thinking skills, excellent communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in electronic medical records and other healthcare software.
This position requires frequent travel to patients' homes and may involve working in a fast-paced, dynamic environment. The ideal candidate will be able to work autonomously and make sound decisions in a variety of situations.
Preferred Qualifications- Home Health Experience: Prior experience in home health, hospice, or infusion preferred.
- OASIS and PDGM Experience: Prior experience with OASIS and PDGM preferred.
