About the Role:- Perform comprehensive assessments and develop individualized care plans to meet the unique needs of each patient.
- Collaborate with interdisciplinary teams, including discharge planners and social service workers, to ensure seamless care transitions.
- Conduct regular home visits to evaluate the quality of care and provide education to patients and their families.
- Participate in ongoing assessment and review of patient progress, making adjustments to care plans as needed.
- Current licensure as a Registered Nurse in the State of Minnesota.
- Minimum of two years of current nursing experience, with a focus on case management.
- Preferred: Community health and medical-surgical experience.
- Competitive annual salary range: $89,905 - $106,233.
- $6,000 hiring bonus.
- Mileage reimbursement and company car program.
- PTO, holiday pay, medical, dental, vision insurance, 401(k), HSA with company contribution, and more.
- 100% clinical license reimbursement.
- CEU reimbursement up to $400 per year and two days of CEU time off.
