Job Title: RN Case Manager

As a Registered Nurse Case Manager, you will play a vital role in delivering high-quality care and improving the lives of our patients.

About the Role:
  • Perform comprehensive assessments and develop individualized care plans to meet the unique needs of each patient.
  • Collaborate with interdisciplinary teams, including discharge planners and social service workers, to ensure seamless care transitions.
  • Conduct regular home visits to evaluate the quality of care and provide education to patients and their families.
  • Participate in ongoing assessment and review of patient progress, making adjustments to care plans as needed.
Requirements:
  • Current licensure as a Registered Nurse in the State of Minnesota.
  • Minimum of two years of current nursing experience, with a focus on case management.
  • Preferred: Community health and medical-surgical experience.

  • Competitive annual salary range: $89,905 - $106,233.
  • $6,000 hiring bonus.
