Job Title: Licensed Practical Nurse - Home Health

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team in Redding, CA. As a LPN, you will play a vital role in providing high-quality patient care and support to our patients in the comfort of their own homes.

Job Summary:

The LPN will be responsible for providing skilled nursing care to patients in their homes, working closely with the healthcare team to develop and implement individualized care plans. This is a full-time, part-time, and PRN opportunity.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Conduct patient assessments, identify needs, and develop plans to address those needs.
  • Collaborate with the healthcare team to ensure seamless care transitions and coordination.
  • Provide education and support to patients and their families on disease management, medication use, and self-care techniques.
Requirements:
  • Current LPN licensure in the state of California.
  • One year of prior professional nursing experience as an LPN.
  • CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
