**Job Summary:**
We are seeking a registered nurse (RN) or licensed vocational nurse (LVN) to provide high-quality clinical home care to children with complex medical needs. As a pediatric home health nurse, you will work closely with our interdisciplinary team to develop and implement individualized care plans that promote the health and well-being of our patients.

**Responsibilities:**
* Provide skilled nursing care to pediatric patients in their homes
* Develop and implement individualized care plans in collaboration with our interdisciplinary team
* Conduct thorough assessments of patients' medical needs and develop plans to address those needs
* Collaborate with other healthcare professionals to ensure seamless care transitions
* Educate patients and their families on self-care and disease management
* Maintain accurate and detailed records of patient care

**Requirements:**
* Current RN or LVN license in the state of Texas
* Current CPR certification
* 1+ year of experience in pediatric home health or a related field
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
