Job Summary:

We are seeking a skilled LPN to join our team at VNA of Care New England. As a Home Health Clinician, you will be responsible for managing a caseload of patients, coordinating interdisciplinary care, and monitoring treatment to achieve positive therapeutic outcomes.

Key Responsibilities:

  • Manage a caseload of patients and coordinate interdisciplinary care
  • Monitor and adjust treatment plans to achieve positive therapeutic outcomes
  • Identify opportunities for process improvement and implement changes as needed
  • Collaborate with healthcare teams to ensure seamless care transitions

Requirements:

  • Equivalent education required for RI LPN licensure
  • Valid RI LPN license
  • A minimum of one year progressive clinical experience
  • Previous experience in an acute care or Home health care environment setting preferred
  • Valid CPR certification
  • Valid driver's license and automobile liability insurance
