Key Responsibilities- Conduct comprehensive assessments of patients and develop personalized care plans to address their unique needs.
- Collaborate with our interdisciplinary team, including physicians, nurses, and social workers, to ensure seamless care coordination and communication.
- Prescribe medications and treatments as needed, and provide education to patients and their caregivers on disease management and self-care.
- Participate in ongoing quality improvement initiatives to ensure the highest standards of care and patient satisfaction.
- Stay up-to-date on relevant medical and nursing research, technology, and best practices to provide the most effective care possible.
- Current state RN license and NP license or ability to obtain before start date.
- Completed Nurse Practitioner accredited program before start date.
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
- Knowledge of CPT, ICD-10, and HCPC codes, as well as clinical standards of care and NCQA requirements.
