About the Role

We are seeking a skilled Medical Social Worker to join our team at BAYADA Home Health Care. As a Medical Social Worker, you will play a vital role in providing exceptional support to our clients and their families.

Key Responsibilities
  • Conduct home visits to assess clients' social and emotional needs
  • Develop and implement individualized care plans to address clients' unique needs
  • Collaborate with the multidisciplinary team to ensure comprehensive care
  • Provide education and support to clients and their families on community resources and services
  • Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet
Requirements
  • Licensed Social Worker in the state of Hawaii
  • Master's degree in Social Work (MSW) with at least 2 years of community-based experience
  • Accreditation by the Council on Social Work Education
  • Strong computer skills and experience with electronic medical records (EMR)
  • Ability to travel to cases as assigned
