Role Description

The Licensed Practical Nurse (LPN) at Evergreen Health provides essential care services to patients within our Primary and Specialty Care Services. This role involves collaborating with healthcare professionals to ensure a patient-focused approach to care.

Main Responsibilities:

  1. Assess and Plan Patient Care: Evaluate patient needs and develop effective care plans, ensuring timely and accurate documentation.
  2. Collect and Record Health Information: Gather and document health information, leveraging HEALTHeLINKTM to access medical records as necessary.
  3. Communicate Effectively: Assist patients over the phone and direct them to relevant resources. Complete intake processes, documenting patient interactions and histories in Medent.
  4. Support Laboratory and Administrative Functions: Assist with diagnostic tests, lab work, and administrative tasks related to the HIV/STI/Hepatitis screening program.
  5. Procedure Assistance: Collaborate with physicians, nurse practitioners, and registered nurses to complete procedures like pap smears, anoscopy, and ear lavage.

Essential Requirements:

  • Active LPN license and BCLS certification
  • At least one year of medical practice experience, preferably in HIV/AIDS or infectious disease settings
  • Prior phlebotomy experience necessary
  • Strong written and verbal communication skills
  • Proficiency in MS Office and electronic charting systems
  • Integrity and responsibility in handling PHI, adhering to HIPAA guidelines
  • Experience working with diverse populations and understanding of cultural competency
