Job Summary

UnityPoint Health is seeking a compassionate and skilled Home Care/Hospice Aide to join our team. As a Home Care/Hospice Aide, you will provide personal care, homemaker services, and companionship to hospice patients under the direction of the RN case manager.

Responsibilities
  • Provide personal care and homemaker services to hospice patients, including bathing, grooming, and light housekeeping.
  • Assist patients with toileting and incontinence care, and perform other tasks as assigned by the RN case manager.
  • Complete telephony care plans accurately and in a timely manner, while being compliant with hospice program policies and procedures.
  • Establish and maintain successful interpersonal relationships with patients and their families, while maintaining personal and professional boundaries.
  • Report changes in patient medical, nursing, and social needs to the RN case manager as they relate to the plan of care.
Requirements
  • High School diploma or GED.
  • Completion of a Nursing Assistant Training Program of a minimum of 75 hours.
  • Valid Iowa and/or Illinois certification.
  • Proficiency with computer applications, including Excel, Word, and Access.
  • Valid driver's license when driving any vehicle for work-related reasons.
  • Age-specific care and reporting of Dependent Adult and Child Abuse training within one month of employment.
  • CPR Certification.
  • Valid Mandatory Reporter course completion by state requirement.
  • Valid, licensed driver, with automobile insurance in accordance with state and/or organizational requirements.
Preferred Qualifications
  • Person-Centered Care (PCC) course completion within the first 12 months of hire.
  • Annual completion of competency validation activities.
  • 12 hours of Aide-Specific Training per year.
  • Mandatory Report: Child & Dependent Adult Abuse.
  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with re-certification.
