Core Responsibilities:
In collaboration with a physician, you will expand individualized primary healthcare services by diagnosing and managing various acute or chronic health conditions.
- Conduct thorough patient assessments through observation, interviews, and examinations.
- Order, perform, and interpret diagnostic tests.
- Provide preventative health assessments, screenings, immunizations, and direct patient care.
- Refer patients to other healthcare providers or community resources as necessary.
- Educate patients and their families on preventative health, treatment options, and available community resources.
- Document healthcare delivery and nursing processes in accordance with specialty standards and network policies.

Qualifications:
Licensure/Certification/Registration:
- BLS Provider certification required within 30 days of hire.
- One or more of the following credentials required:
- Nurse Practitioner certification from ANCC, Indiana Board of Nursing, or NCC within 180 days of hire.
- Registered Nurse certification from ANCC or Indiana Board of Nursing prior to hire.
Education:
- Master's degree in Nursing is required.
