Join Our Team as a Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at AcuteCare Health System LLC. As a Nurse Practitioner, you will play a vital role in providing high-quality, patient-centered care to our elderly population.

About the Role

As a Nurse Practitioner, you will be responsible for providing primary medical management in collaboration with our physicians. You will conduct initial, semi-annual, and unscheduled assessments and reassessments, perform additional physical assessments as needed, and participate in the development and implementation of participant plans of care in collaboration with our interdisciplinary team.

Responsibilities
  • Provide primary medical coverage in collaboration with PACE physicians
  • Perform initial, semi-annual, and unscheduled assessments and reassessments in collaboration with the physician
  • Participate in the development and implementation of participant plans of care as part of and in collaboration with the interdisciplinary team
  • Function as a member of the interdisciplinary team, maintaining regular attendance at and participating in team meetings
  • Communicate participant changes, collaborate on care planning decisions, and coordinate 24-hour care delivery
  • Integrate the primary care treatment plan into the plan of care developed in collaboration with the interdisciplinary team
  • Interact with team members to meet emergent and acute needs of participants
  • Participate in discharge planning for acute and long-term placement
  • Evaluate and treat participants during acute illness
  • Manage participants' chronic illnesses and conditions in collaboration with the physician(s)
Requirements

To be successful in this role, you will need:

  • A Master's degree in clinical nursing from an accredited Nurse Practitioner program
  • 3-5 years of nursing experience, with Nurse Practitioner experience preferred and 1 year of experience with a frail or elderly population
  • RN licensure in the perspective state and a Family certification from a Nurse Practitioner program
