Discharge Planner

The Pavilion Behavioral Health System is seeking a highly skilled Discharge Planner to join our team. As a Discharge Planner, you will play a critical role in ensuring a smooth transition for patients from our inpatient and partial hospitalization programs to their next level of care.

• Coordinate clinical staffings and make collateral contacts to arrange discharge plans and linkage with aftercare services.
• Provide clinical services to psychiatric inpatient and partial hospitalization patients and their families.
• Develop and implement discharge plans that meet the unique needs of each patient.
• Collaborate with multidisciplinary teams to ensure seamless transitions and optimal patient outcomes.
• Document patient progress and maintain accurate records.

• Bachelor's Degree in a human services field (counseling, psychology, social work) required; Master's degree preferred.
• Previous professional experience in a behavioral health setting preferred.
• Excellent organizational and communication skills.
