Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our adult and geriatric clients in their homes. As a member of our multidisciplinary healthcare team, you will work closely with nurses and other healthcare professionals to develop and implement individualized care plans that promote our clients' health, safety, and independent living.

Responsibilities:
  • Make home visits to clients in designated geographic territories.
  • Perform diagnostic tests and measurements to assess clients' mobility, strength, and balance.
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
  • Educate and instruct clients and their families on rehabilitative care and activities necessary for independent living.
  • Accurately document observations, interventions, and evaluations in our state-of-the-art electronic medical records system.
Requirements:
  • A current Maryland Physical Therapist license.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills and prior experience with electronic medical records (EMR) are preferred.
