We are seeking a highly skilled Registered Nurse to join our team at Atrium Health, where you will play a vital role in delivering exceptional patient care and supporting our healthcare teams.
Key Responsibilities- Collaborate with physicians and advanced care practitioners to develop patient-centric discharge plans, addressing barriers to discharge and promoting efficient workflow.
- Coordinate patient evaluations, testing, management, and follow-up care in collaboration with the attending physician and healthcare team.
- Round with physicians and serve as a liaison between healthcare teams, patients, and family members to ensure seamless care transitions.
- Promote patient and family education, supporting treatment plans and educational needs to achieve continuity of care.
- Ensure patients have scheduled follow-up appointments prior to discharge and participate in performance improvement activities.
- Assist with prioritizing and assigning patients to correct providers and coordinate care outside of the hospital to prevent readmissions and admissions.
- Serve as a resource to clinical staff, addressing related issues and providing support with direct patient care services.
- Respond to patient and care provider inquiries to facilitate care transitions.
- Follow up on pending labs, procedures, and tests to ensure timely completion.
- Graduate from an accredited School of Nursing, with a BSN preferred.
- Minimum 3 years of nursing experience, with ICU or ED experience preferred.
- Current Basic Life Support (BLS) for Healthcare Provider certification from the American Heart Association.
- Current RN license or temporary license as a Registered Nurse Petitioner in the state of employment.
