Job Summary

We are seeking a highly skilled Speech Language Pathologist to join our team at Evolve Therapy Services. As a Speech Language Pathologist, you will play a critical role in providing high-quality speech therapy services to our patients.

Key Responsibilities
  • Develop and Implement Treatment Plans

Design and implement individualized treatment plans to address the unique needs of each patient. This will involve conducting thorough assessments, setting realistic goals, and developing effective interventions to promote communication and swallowing skills.

Provide Quality Speech Therapy Services

Deliver high-quality speech therapy services to patients, including individual and group sessions, to help them achieve their communication and swallowing goals. This will involve using evidence-based practices and staying up-to-date with the latest research and techniques in the field.

Collaborate with Interdisciplinary Team

Work closely with our interdisciplinary team, including nurses, occupational therapists, and physical therapists, to ensure comprehensive care and coordination of services. This will involve communicating effectively, sharing information, and working together to achieve patient-centered outcomes.

Document Patient Progress

Accurately and thoroughly document patient progress, including treatment plans, progress notes, and discharge summaries. This will involve using our electronic medical record system to ensure timely and accurate documentation.

Participate in Quality Improvement Initiatives

Contribute to quality improvement initiatives, including data collection, analysis, and implementation of evidence-based practices. This will involve staying up-to-date with the latest research and best practices in the field and participating in ongoing education and training.

Requirements
  • Master's Degree in Speech Language Pathology

A Master's degree in Speech Language Pathology from an accredited program is required.

Certificate of Clinical Competency

A Certificate of Clinical Competency from the American Speech-Language-Hearing Association (ASHA) is required.

Active SLP License

An active SLP license in the state(s) of practice is required.

Ongoing Continuing Education

Ongoing continuing education and professional development to maintain current licensure and certification is required.
