Join Our Hospice Team as a Spiritual Care Counselor

We are seeking a compassionate and experienced Hospice Spiritual Care Counselor to join our extraordinary care team at Residential Home Health and Hospice. Key Responsibilities:
  • Conduct spiritual assessments of patients and families/caregivers in the hospice program and ensure timely delivery of appropriate services.
  • Develop individualized plans of care by participating in comprehensive assessments to meet identified spiritual needs.
  • Provide direct spiritual care to patients and families/caregivers.
  • Serve as a liaison and support to community chaplains and spiritual care counselors.
  • Provide consultation, education, and support to the interdisciplinary group on spiritual care.
  • Plan periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
  • Participate in quality assessment performance improvement teams and activities.
Requirements:
  • Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree) preferred.
  • Hospice experience preferred.
