Key Responsibilities- Delivering nursing care in post-acute care settings
- Adhering to care plans and ensuring accountability
- Monitoring vital signs and reporting findings to the nursing team
- Observing residents' conditions and notifying staff of any concerns
- Assisting residents with daily living activities (ADLs) such as grooming and feeding
- Maintaining a clean and sanitary environment for residents
- Facilitating communication between residents and healthcare staff
- Providing companionship and emotional support to residents
- Engaging with family members and addressing their inquiries
- Safely assisting residents with mobility and repositioning
- Current certification as a Patient Care Assistant
- Understanding of medical terminology and basic nursing procedures
- Strong observational skills and attention to detail
- Excellent interpersonal skills for effective communication
- Ability to lift at least 50 lbs
