Renal Transplant Coordinator

Mass General Brigham is seeking a highly skilled and compassionate Renal Transplant Coordinator to join our team. As a key member of our transplant program, you will play a vital role in ensuring the seamless care of our patients undergoing renal and pancreas transplants.
Job Summary:- Coordinate the care of patients undergoing renal and pancreas transplants, including pre-transplant evaluation, post-transplant follow-up, and ongoing management of complex medical conditions.
- Collaborate with multidisciplinary teams, including transplant surgeons, nephrologists, and other healthcare professionals to ensure comprehensive and coordinated care.
- Develop and implement individualized care plans for patients, taking into account their unique needs and circumstances.
- Provide education and support to patients and their families, empowering them to take an active role in their care and recovery.
- Stay up-to-date with the latest developments in renal and pancreas transplantation, including advances in immunosuppression, surgical techniques, and post-transplant care.
- Assess patients' medical and psychosocial needs, identifying areas for intervention and developing strategies to address them.
- Coordinate with transplant surgeons, nephrologists, and other healthcare professionals to ensure timely and effective care.
- Develop and implement individualized care plans, incorporating input from patients, families, and healthcare teams.
- Provide education and support to patients and their families, addressing topics such as immunosuppression, medication management, and post-transplant care.
- Collaborate with social workers, financial coordinators, and other support staff to ensure comprehensive care and address patients' non-medical needs.
- Graduate of an approved school of nursing with current registration in Massachusetts.
- Minimum two years of experience in renal medicine-focused patient care, with a strong background in transplant coordination.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong focus on patient-centered care.
- Knowledge of renal and pancreas transplantation, including immunosuppression, surgical techniques, and post-transplant care.
