Healthcare Navigator for Patient Care Coordination

The Diverse Lynx is seeking a highly skilled Care Manager to join our team. As a key member of our healthcare staff, you will be responsible for coordinating patient care and ensuring timely transitions to appropriate levels of care.
This role requires strong communication and interpersonal skills, as well as the ability to work collaboratively with physicians, nurses, social workers, and other healthcare professionals to achieve optimal health outcomes for patients.
About the Role
As a Care Manager, you will:
- Develop and implement individualized care plans that address the unique needs and goals of each patient
- Collaborate with interdisciplinary teams to ensure seamless transitions between levels of care
- Provide education and support to patients and their families to promote self-management and empowerment
- Maintain accurate and up-to-date documentation in electronic health records (EHRs)
- Participate in quality improvement initiatives to enhance patient safety and satisfaction
Requirements
To be successful in this role, you will need:
- A minimum of 2 years of experience in case management or a related field
- Strong knowledge of medical terminology and billing processes
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
Benefits
We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, as well as opportunities for professional growth and development.
