">We are seeking a dedicated Licensed Practical Nurse to join our team at a skilled nursing facility in Bennington, VT. As a valued member of the care team, you will play a crucial role in helping patients with medically complex conditions regain their quality of life.

">- ">
- Assist residents with daily bath, dressing, grooming, dental care, bowel and bladder functions, preparation for medical tests and exams, ear and eye care, and positioning in and out of beds, chairs, bathtubs, etc.">
- Provide nursing functions as directed by supervisor, including daily perineal care, catheter care, turning residents in bed, bathing, measuring and recording temperature, pulse, and respiration, weighing and measuring residents, performing restorative and rehabilitative procedures, and observing and reporting skin breakdowns.">
- Prepare residents for meals and snacks, identify food arrangements, assist in feeding residents as needed, record food and fluid intake, and perform after-meal resident care as needed.">
- Maintain resident confidentiality, treat residents with kindness, dignity, and respect, know and comply with Resident's Rights rules, and promptly report all resident complaints, accidents, and incidents to the supervisor.">
- Utilize nursing knowledge and skills in the safe implementation of basic preventive therapeutic, and rehabilitative nursing care of assigned patients/families, as evidenced by documentation and observation of positive patient care outcomes.">
- Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards, as observed by the registered nurse.">
- Assist in data collection on the Admission Data Record.">
- Contribute to the development of the patient/significant other plan of care based on identified patient problems/needs.">
- Carry out physician orders accurately, directly, and indirectly.">
- Assist in the evaluation of the effectiveness of the nursing care administered relative to the patient/significant other identified problems/needs.">
- Document pertinent observations and interventions on appropriate records.">
- Perform selected tasks related to patient care and the patient's environment in a safe manner.">
- Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes.">
- Perform limited IV therapy procedures on patients receiving peripherally inserted intravenous fluids, as delegated by and observed by the RN.">
- Maintain and coordinate communication among patients/significant others, staff, and physicians, as evidenced within medical records, client, and health care team feedback.">
- Give punctual change of shift report consisting of pertinent and relevant information for assigned patients.">
- Provide patient/significant other health teaching, as delegated by and observed by the RN.">
- Use available written and AV materials specific to patient condition or disease.">
- Utilize appropriate teaching methods, such as one-on-one and group instruction, demonstration, or discussion.">
- Participate in staff development programs and meetings, as requested.">
Requirements

- Graduate of a nationally approved nursing program is required.">
- Maintain current BLS/CPR certification from either the American Heart Association (AHA) or the American Red Cross (ARC).">
- Must be currently licensed by the State Board of Nursing.">
