Job Summary

In accordance with agency policy and state/federal regulations, provides independent, age-appropriate physical therapy to patients in their place of residence. Teaches patient/family and caregivers preventative and rehabilitative procedures. Develops and coordinates the plan of care and provides case management.

Key Responsibilities
  1. Evaluates patients and interprets evaluation findings to determine the nature and extent of dysfunction
  2. Detects early warning signs of complications including medical assessments of skin integrity and mentation
  3. Ability to differentiate emergent versus non emergent patient care status and escalates appropriately
  4. Providing initial and ongoing assessment to determine level of functioning
  5. Evaluate the patient comprehensively, not just body part or functional ability
  6. Establishes treatment goals and plans treatment to achieve established goals
  7. Initiates discharge planning for physical therapy
  8. Implements the plan of care through direct treatment of the patient other health care personnel as appropriate
  9. Provides advice and education to patient and family
  10. Regularly reassesses clinical symptoms to determine effectiveness of treatment, progress of patients towards goals, and the need for modifying goals and treatment
  11. Discharges patient from physical therapy when the patient has met their treatment goals
  12. Provides accurate, timely documentation of physical therapy patient management
  13. Participates in appropriate and approved educational and clinical research activities in physical therapy
  14. Completes OASIS documentation according to guidelines
  15. Coordinating with pharmacy, inpatient physical therapy, outpatient departments
  16. Assists in the development, implementation, and review of departmental patient care programs
  17. Performs other duties as needed in the areas of staff education, peer reviews, departmental operations, and departmental projects
  18. Supervises the work of Home Health Aides and Physical Therapy Assistant according to regulation and agency policy
  19. Demonstrates on-going progress toward meeting the criteria for professional practice
  20. Consistently supports compliance and the Principles of Responsibility (Kaiser Permanentes Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanentes policies and procedures
Requirements
  • Minimum of two (2) years experience in physical therapy
  • Graduate of an accredited physical therapy curriculum with a Bachelors, Masters or Doctoral degree with certification in Physical Therapy or foreign-trained equivalent
  • Driver's License (California)
  • Physical Therapist License (California)
  • Basic Life Support
  • National Provider Identifier
  • Clinically skilled to detect early warning signs of complications including medical assessments of skin integrity, mentation
  • Ability to differentiate emergent versus non emergent patient care status
  • Ability to look at patient holistically, not just body part or functional ability
  • Competency to remove staples/sutures in the home
  • Commitment to Service Orientation
  • Effective written and oral communication skills (in English)
  • Good interpersonal skills
  • Must be able to work in a Labor/Management Partnership environment
  • Clear driving record for the past two (2) years and proof of automobile insurance as required by law
Preferred Qualifications
  • Experience in an automated clinical information system preferred
  • 1-year experience in home health agency preferred
  • OASIS Certification
