Key Responsibilities:- Provide comprehensive nursing care to pediatric patients in their homes, under the direction of a registered nurse.
- Participate in the development of individualized care plans, collaborating with interdisciplinary teams to ensure seamless care delivery.
- Deliver health education to patients and caregivers, empowering them to take an active role in their care.
- Accept and enter verbal and written orders from physicians, ensuring timely and accurate documentation.
- Maintain open communication with physicians, patients, family members, and staff, addressing concerns and reporting changes in patient condition.
- Active LPN license in the state of Connecticut or compact license.
- Minimum 6 months of pediatric experience, with a preference for LPN home health experience.
- Reliable transportation to perform job duties.
- Dedication to delivering high-quality patient care.
