Trusted Resource Associates (TRA) is seeking a skilled Local Contract Nurse RN Case Management professional to join our team.

The RN Case Manager is responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient's resources. This position manages medical necessity process for accurate and timely payment for services which may require negotiation with a payer on a case by case basis.

Responsibilities
  • Coordinate care with healthcare providers to ensure seamless transitions and optimal patient outcomes
  • Develop and implement individualized care plans to meet patient needs
  • Collaborate with payers to negotiate payment for services
  • Ensure compliance with regulatory requirements and TRA policies
Requirements
  • Must have at least 2 years acute care experience and at least one year of Case Management experience in an Acute Care Discharge environment
  • Alabama State License
  • BLS
