We are seeking a skilled Healthcare Navigator to join our team at Care Alliance Health Center. As a key member of our care coordination team, you will play a vital role in ensuring our patients receive comprehensive and compassionate care.

Job Summary

The Care Coordination Specialist will work closely with patients, healthcare providers, and community resources to coordinate medical and social care services. This includes assessing patients' needs, developing individualized service plans, and providing supportive care coordination and case management for chronic illnesses.

Key Responsibilities
  • Medical- and Social Case Management: Assess patients' medical and social needs, and develop individualized service plans to meet those needs.
  • Chronic Illness Management: Provide supportive care coordination and case management for patients with chronic illnesses.
  • Healthcare Resource Linkage: Link patients with education and support opportunities, such as diabetes education and support groups.
  • Referral and Consultation: Facilitate internal and external referrals and consults, and follow up after scheduled referrals/appointments.
  • Transportation Assistance: Coordinate transportation assistance for referral appointments.
  • Community Resource Development: Establish positive relationships with healthcare resources and community organizations to link patients with a matrix of services that promote self-sufficiency.
Requirements
  • Education: High School Diploma, Associate or Bachelor's degree in Public Health, Social Work, or a related field.
  • Experience: 2+ years of experience as a Patient Navigator, Care Coordinator, or Case Manager, with 2+ years of medical and/or resource coordination experience.
  • Skills: Ability to work compassionately with a diverse population, plan, organize, and complete associated paperwork promptly, and maintain confidentiality.
