Job Summary:

As a skilled rehabilitation staff nurse at LPNT West, you will play a vital role in assisting individuals with disabilities and chronic illnesses to attain and maintain maximum function. You will work closely with clients and their families to provide a therapeutic environment for development and adaptation to an altered lifestyle.

Key Responsibilities:
  • Complete referrals and take physician's verbal orders in a timely manner.
  • Perform physical assessments, evaluate patient needs, and consult with physicians to develop and document care plans.
  • Assess the need for additional health or ancillary services and refer patients as necessary.
  • Involves patients and/or family in developing and revising individualized care plans, including necessary teachings and realistic goals.
  • Coordinate the multidisciplinary care team and plan for patient care, documenting admission, discharge, and other policies as needed.
  • Provide clear, written, and verbal assignments for Home Health Aide and LPN care, according to the plan of care.
  • Establish and modify goals, provide progress notes to physicians, and initiate preventive, rehabilitative, comfort, and pain management nursing procedures.
  • Explain diagnoses and treatment plans to patients and families, consistent with physician orders and plan of care.

Requirements:
  • Graduate of an accredited nursing program.
  • Current nursing license, in good standing.
  • Minimum of one year current rehabilitation/skilled nursing experience.
  • Current AHA BLS, ACLS if unit consists of Telemetry.
