Cantex is seeking a skilled and compassionate Registered Nurse to join our Hospice team in Elysian Lewisville - Lancaster. As a Hospice Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families during a challenging time.
Key Responsibilities- Administer nursing care to patients of all ages in their place of residence, coordinating care with the interdisciplinary group, patient, family, and referring agency.
- Assume responsibility for the coordination of care, ensuring continuity and quality of service.
- Conduct initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs.
- Apply specific criteria for admission and re-certification to hospice care, establishing appropriate levels of care and patient eligibility.
- Implement and develop plans of care to ensure quality and continuity of care, recommending revisions as necessary.
- Consult with and educate patients and families regarding disease processes, self-care techniques, end-of-life care, nutrition, and dietary needs.
- Determine scope and frequency of services needed based on acuity and patient/family needs.
- Assess caregivers' ability to meet patients' immediate needs upon admission and throughout care.
- Initiate preventive and rehabilitative nursing procedures as needed.
- Apply concepts of infection control and standard precautions in coordinating and performing patient care activities.
- Provide clinical directions to Hospice Aides and LPNs/LVNs to ensure quality and continuity of service.
- Prepare clinical and progress notes that demonstrate progress toward established goals.
- Inform physicians and other personnel of changes in patients' needs and outcomes of interventions.
- Monitor assigned cases to ensure compliance with requirements of third-party payors.
- Demonstrate commitment, professional growth, and competency.
- Promote Agency philosophy and administrative policies.
- Perform on-call responsibilities and provide on-call service to patients and families as assigned.
- Provide effective communication to patients, families, team members, and other healthcare professionals.
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current license as a Registered Nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
- 2 years of experience as a Registered Nurse in a clinical care setting, preferably in hospice care.
- Nursing skills as defined by generally accepted standards of practice.
- Good interpersonal skills.
- Specialized skills preferred in palliation and end-of-life care.
- Proof of current CPR.
- Ability to read, write, and comprehend English.
- Reliable transportation and valid and current auto liability insurance.
- Ability to work in patients' homes in various conditions, with possible exposure to blood and bodily fluids and infectious diseases.
- Ability to work flexible schedules, travel locally, and some exposure to unpleasant weather.
- PRN emergency call.
