At UnitedHealth Group, we are committed to making healthcare better for everyone. As a Nurse Practitioner or Physician Assistant on our Home-Based Medical Care team, you will play a vital role in delivering high-quality, patient-centered care to our most medically vulnerable members.

Key Responsibilities:

  • Perform comprehensive assessments and develop personalized care plans for patients
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Prescribe diagnostics and interventions to prevent unnecessary acute admissions
  • Educate patients and their families on disease management and wellness strategies
  • Participate in quality improvement initiatives to enhance patient outcomes

Requirements:

  • Completion of a Nurse Practitioner or Physician Assistant accredited program
  • Current state RN/NP or PA license
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)

Preferred Qualifications:

  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
  • Knowledge of long-term care, community resources, and cost-effective alternatives
  • Knowledge of CPT, ICD-10, and HCPC codes
