Job Summary

We are seeking a highly skilled and dedicated After Hours Registered Nurse to join our team. As an After Hours RN, you will be responsible for providing high-quality nursing care to patients in our home health care program, including hospice and home care services.

Key Responsibilities
  • Obtain reports at the beginning of each shift and update records at the end of each shift.
  • Respond to beeper calls in a timely manner during shift hours.
  • Triage calls and make scheduled and emergent visits to patients as needed.
  • Use the nursing process to provide physical, psycho-social, and spiritual care to patients and their families.
  • Intervene with patients and families in crisis situations and act as patient advocate.
  • Perform clinical nursing tasks, including phlebotomy, subcutaneous devices, medication administration, and other nursing care as needed.
  • Develop and implement plans of care for patients under the order of a physician.
  • Assume on-call responsibility as scheduled after regular business hours for all home care/home hospice patients.
  • Return calls within 15 minutes and visit patients as necessary.
  • Document all visits, phone calls, and admissions within 24 hours.
  • Complete accurately and submit weekly all itinerary forms and risk-indicator incident sheets.
  • Coordinate patient transfers to IPU, including ambulance transport, sending reports, and notification of physicians.
Requirements
  • Current RN licensure in the state of Maryland.
  • Experience in home health care or hospice care preferred.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work varied shifts, including weekends and holidays.
