Key Responsibilities:
  • Conduct Comprehensive Assessments: Evaluate patients' medical needs and develop personalized care plans to ensure optimal outcomes.
  • Manage Complex Medical Situations: Provide expert care to patients with acute medical conditions or chronic illnesses, including testing, diagnostics, and interventions.
  • Collaborate with Interdisciplinary Teams: Work closely with healthcare professionals to ensure seamless care coordination and communication.
  • Provide Advanced Illness and End-of-Life Discussions: Offer empathetic support and guidance to patients and their families during challenging times.
  • Counsel and Educate Patients and Caregivers: Empower patients and their loved ones with the knowledge and skills needed to manage their health effectively.
Requirements:
  • Graduate of an Accredited Master's or Doctoral Nursing Program: Possess a strong foundation in nursing principles and practices.
  • Active and Unrestricted License: Hold a current, active license to practice nursing in the state where you reside.
  • National Board Certification: Be certified as a Nurse Practitioner through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
  • Current DEA Licensure/Prescriptive Authority: Possess current, active DEA licensure or prescriptive authority, or be able to obtain it post-hire, per state regulations.
  • Ability to Gain a Collaborative Practice Agreement: Be able to secure a collaborative practice agreement, if required in your state.
  • Physical Requirements: Meet the physical demands of the job, including lifting, navigating stairs, and working in various dwelling conditions.
  • Access to Reliable Transportation: Possess a reliable vehicle for travel to patient locations.
