About the Role

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. As a key member of our clinical staff, you will play a critical role in improving healthcare outcomes for our patients.

Job Overview

This is a part-time position that offers flexible scheduling and competitive compensation. You will be responsible for conducting comprehensive health assessments in the comfort of our patients' homes, using your expertise to identify potential health risks and develop personalized care plans.

Responsibilities
  • Conduct home visits to assess patients' health status and identify potential risks
  • Develop and implement personalized care plans to address identified health concerns
  • Collaborate with our interdisciplinary team to ensure seamless coordination of care
  • Provide education and support to patients and their families on healthy lifestyle choices and disease management
Requirements

To be successful in this role, you will need:

  • A Master's Degree in Nursing or a related field
  • Current RN licensure and NP certification in a relevant specialty
  • Experience working as a Nurse Practitioner in a clinical setting
  • Strong communication and interpersonal skills
  • The ability to work independently and as part of a team
