Job Summary

We are seeking an experienced Registered Nurse (RN) to serve as an MDS Coordinator in our Long Term Care Unit at UH Lakewood Medical Center. As an MDS Coordinator, you will be responsible for managing Resident care as related to the Minimum Data Set (MDS) and care plans. You will work closely with our interdisciplinary team to ensure that Residents receive high-quality care that meets their individual needs.

Responsibilities
  • Manage Resident care as related to the MDS and care plans in accordance with HFCA regulations and guidelines, state regulations, and the mission of Truman Medical Centers.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans for Residents.
  • Conduct regular assessments of Residents' needs and update care plans accordingly.
  • Ensure accurate and timely completion of MDS records and care plans.
  • Promote Resident quality of life and quality of care through effective communication and collaboration with the care team.
Requirements
  • Bachelor's degree in Nursing from an accredited college or university.
  • Curren license to practice as a RN in the State of Missouri.
  • Three years of direct care experience, one year long term care experience.
  • Knowledgeable of current standards of nursing practice and procedures, as well as laws, regulations, and guidelines that pertain to long term care.
  • Ability to apply nursing techniques to specific patient situations.
  • Excellent communication and interpersonal skills.
  • Able to work independently and collaboratively as part of a team.
Estimated Salary Range

$64,000 - $84,000 per year, depending on experience.
