We are seeking a skilled and compassionate Physical Therapist to join our team at JAG Physical Therapy. As an Orthopedic Rehabilitation Specialist, you will be responsible for assessing patients and designing effective treatment plans to facilitate their healing and functional independence.

- You will assess patients and design effective treatment plans to facilitate their healing and functional independence.
- You will collect and record patient care data that follows format and guidelines of the patient care service, regulatory agencies, and third-party payers.
- You will prepare for evaluations in accordance with department policies/procedures and standards of practice.
- You will select and administer tests and measures.
- You will synthesize evaluation results to formulate the diagnosis, goals, prognosis, and plan of care.
- You will establish treatment plans based on evaluation results, goals, and best practice.
- You will collaborate with interdisciplinary teams, patients, and patient support systems.
- You will identify need for consultations and/or refer patients to another healthcare professional or healthcare services.
- You will initiate and complete discharge plans.
- You will provide education/training to patients and/or patient support systems.
- You will provide interventions to patients that are safe, effective, and in compliance with JAG.
- Bachelor's or Master's Degree required; Doctorate degree preferred.
- Licensed Physical Therapist in the state practicing in.
- Completed state-regulated CEU requirement in appropriate state (NY, NJ).
- Valid CPR License.
- Experience/knowledge with EMR software preferred.
- Excellent oral, written communication skills.
- Strong clinical decision-making skills.
- Excellent work ethic and dedication to patient success.
- Basic computer skills.
- Ability to work as a team member and individually with minimal supervision.
- Maintains a positive, professional demeanor at all times.
