Job Title: Travel Nurse/Home Health Care RN

About the Role:

We are seeking a skilled and compassionate Registered Nurse to provide high-quality patient care in various settings, including patient homes, hospitals, long-term care facilities, and assisted living facilities. As a Travel Nurse/Home Health Care RN, you will work collaboratively with the interdisciplinary team to ensure effective and efficient patient care delivery and achievement of desired patient outcomes.

Responsibilities:
  • Deliver patient care in accordance with acceptable standards of care, utilizing knowledge of patient's age, gender, and cultural diversity.
  • Collaborate with other professional disciplines and members of the hospice interdisciplinary team to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes.
  • Provide supervision of the Hospice aides and LPNs, ensuring compliance with State and Federal regulations and agency policy and procedure.
  • Document care in accordance with medical documentation standards and agency policy, standards, and timelines.
  • Participate in peer chart review and other Quality Assessment/Performance Improvement activities as directed.
Requirements:
  • Georgia RN licensure and current BLS certification.
  • Minimum of one (1) year of clinical nursing experience.
  • Ability to work independently and as a member of a team, with excellent customer service and interpersonal skills.
  • Heavy travel throughout the service area, with a valid driver's license and current auto insurance with a minimum of $100,000 liability.
Preferred Qualifications:
  • CHPN certification.
  • Experience with emphasis on care of terminally ill patients, adults in acute care medical and surgical areas/home care.
  • One (1) year hospice experience.
