The Admissions / Referral Manager plays a crucial role in overseeing daily patient referral and intake operations. This position involves the establishment and enforcement of intake policies to ensure a smooth process for incoming patients.

Key Responsibilities
  1. Operational Oversight: Directs all aspects of patient referral and intake, ensuring adherence to state, federal, and CHAP regulations.
  2. Process Improvement: Implements enhanced work methods and procedures to guarantee that patient admissions align with established policies.
  3. Relationship Management: Cultivates and maintains strong relationships with existing and potential referral sources.
  4. Patient Transition: Facilitates a seamless transition for patients to home care by overseeing education, care planning, and coordination with various service providers.
  5. Insurance Coordination: Ensures optimal third-party reimbursement through diligent management of insurance verification and authorization processes.
  6. Pricing Negotiation: Engages with insurance Case Managers and other payers to negotiate service pricing within set financial parameters.

  8. Team Leadership: Recruits, trains, and manages the referral/intake team to ensure high performance.

  11. Strategic Planning: Provides leadership in strategic initiatives, identifying opportunities for service enhancements to meet customer needs.
  12. Contract Compliance: Maintains a thorough understanding of contractual obligations to ensure patient admissions are compliant.
  13. Quality Improvement: Participates in quality assessment and performance improvement initiatives.
  14. Resource Navigation: Assists referral sources in accessing community resources when services are not provided by HospiceCare in The Berkshires.
