Hospice Registered Nurse Case Manager - The RN Case Manager is responsible for planning, organizing, directing, and delivering hospice services tailored to the specific needs of patients and their families.
The RN conducts evaluations of both psychosocial and physical conditions and collaborates with the Interdisciplinary team to create and implement a comprehensive care plan.

Annual Salary Range:
$81,960 - $94,440 based on experience, plus additional compensation for on-call duties.

You will serve a general area within our service region, providing in-home care to patients in various settings (personal residences, nursing facilities, etc.).
The primary territory for this role is in the southern section of the Twin Cities Metro area.Join Interim HealthCare and:

Qualifications:
Graduate from an accredited nursing program.
Licensed as a Registered Nurse in the state of MN.
License must be active and in good standing.
At least twelve (12) months of nursing experience within the last five (5) years, or if lacking recent experience, evidence of current clinical knowledge as deemed appropriate by the DHCS.
If you are a new graduate, you must have 1200 verified hours of employment as a graduate or registered nurse within the last twelve (12) months or proof of completion of a company-approved Preceptor Program.
One (1) year of experience in hospice care is preferred.Proof of a valid driver's license.
Proof of valid auto liability insurance.
