The Licensed Practical/Vocational Nurse plays a crucial role in delivering direct patient care under the guidance of a registered nurse.

Key Responsibilities:

  • Adhere to the established plan of care, providing necessary treatments, and collaborating with team members to achieve positive patient care outcomes.
  • Deliver direct patient care in accordance with the defined Plan of Care and as outlined in the State Nurse Practice Act.
  • Ensure accurate and timely documentation of patient services reflecting the plan of care.
  • Educate patients and their families/caregivers about relevant information concerning diagnosis and care plans.
  • Assist in coordinating hospice services, reporting any identified needs to the interdisciplinary team.
  • Engage in Continuous Care as required.
  • Participate in Interdisciplinary Group meetings to enhance care coordination and discuss team involvement.
  • Contribute to quality assessment and performance improvement initiatives.

Qualifications:

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Preferred prior experience in home health or hospice settings, with a minimum of one year in medical/surgical care.

All personnel are expected to engage in relevant continuing education as requested or mandated by their immediate supervisor.
