Job Title: Per diem Hospice LPN

We are seeking a skilled and compassionate Per diem Hospice LPN to join our team at HealthEcareers - Client. As a Per diem Hospice LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families in need of hospice services.

Job Summary:

The Per diem Hospice LPN will be responsible for providing nursing care and support to patients in their homes, hospitals, or other healthcare settings. This includes assessing patients' physical and emotional needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure comprehensive care.

Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans to meet patients' needs
  • Provide nursing care and support to patients in their homes, hospitals, or other healthcare settings
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document patient information and progress in a timely and accurate manner
  • Participate in interdisciplinary team meetings to discuss patient care and develop care plans
  • Provide education and support to patients and their families on disease management, symptom control, and end-of-life care
  • Perform other duties as assigned by the supervisor or manager
Requirements:
  • Graduation from an accredited school of Practical Nursing
  • Licensure to practice as a Practical Nurse in the State of New Jersey
  • Minimum one-year adult or pediatric medical/surgical clinical experience
  • Ability to comprehend the spoken and written English language
  • Excellent interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Proficiency in operating a computer and electronic medical record systems
Preferred Qualifications:
  • Prior home care or hospice experience
  • Strong nursing assessment and documentation skills
  • Clinical knowledge to support the dying process
  • Ability to serve as a resource to patients and families during times of high stress and volatility
