Responsibilities

  • Evaluate patient needs and develop personalized care plans to address their unique challenges.
  • Provide speech therapy services as outlined in the patient care plan, utilizing your expertise to help patients achieve their goals.
  • Regularly report patient status to our Clinical Supervisor and referring Physician, ensuring seamless communication and collaboration.
  • Document changes to patient care plans, maintaining accurate and up-to-date records.
  • Attend staff meetings and educational in-services to stay current on best practices and industry developments.
  • Perform OASIS time point assessments and submit recertification paperwork in a timely and accurate manner.

Requirements

  • Current State Professional License and driver's license are required for this role.
  • At least 1 year of experience as a home care Speech Therapist is preferred.
  • Proficiency in clinical skills and Microsoft Office is essential for success in this position.
