We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team in the Primary Care Outpatient Clinic. As an LPN, you will play a vital role in providing direct support to the Patient-Aligned Care Team (PACT) and ensuring the smooth flow of clinic operations.
Key Responsibilities- Provide direct patient care, including vital signs, treatments, and medication administration.
- Assist providers with exams and procedures, and prepare patients for medical appointments.
- Collaborate with the PACT team to develop comprehensive health care plans and care management plans for patients.
- Manage clinic flow, including scheduling, patient communication, and coordination of care.
- Participate in patient education and health promotion activities, including disease prevention and management.
- Work closely with other healthcare professionals to ensure seamless care coordination and communication.
- Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC).
- Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, or Commonwealth of the United States, or District of Columbia.
- English language proficiency, as required by 38 U.S.C. 7402(d) and 7407(d).
- Experience working in a primary care outpatient clinic setting.
- Knowledge of patient-centered care principles and practices.
- Ability to work effectively in a team environment and communicate with patients, families, and healthcare professionals.
