The Hospice Care Coordinator ensures the quality and safe delivery of hospice services to clients in their homes, skilled nursing facilities, or assisted living environments.
Our RN Care Coordinators are responsible for developing, implementing, and evaluating care plans, making necessary adjustments to address all client needs.
They coordinate hospice services, ensuring continuity of care with other healthcare professionals while identifying interdisciplinary care requirements.
Care Coordinators utilize assessment, intervention, and educational skills to enhance patient comfort and improve the quality of life for patients and their families.
Key tasks and responsibilities include conducting ongoing assessments of the physical, psychological, social, and spiritual needs of patients and families, providing education, supervision, and counseling to clients and their caregivers regarding care needs, and maintaining accurate documentation of patient and family services.
Qualifications:
A strong commitment to serving older adults with a focus on end-of-life care.
Registered nurse with current licensure in the state(s) of practice.
A minimum of one year of nursing experience is preferred.
Experience in critical care/oncology, long-term care, or medical/surgical settings with a solid understanding of pain and symptom management is preferred.
Hospice/Home Care experience is beneficial but not mandatory.
Excellent interpersonal skills.
Ability to work with patients and caregivers in various settings.
Ability to collaborate effectively as part of an Interdisciplinary Team.
Strong assessment skills, with experience in symptom management preferred.
Strong oral and written communication skills.
Proficiency in computer skills is advantageous.
Reliable transportation, a valid driver's license, and current auto insurance are required.
Demonstrated alignment with PHS's mission and operational philosophies.
Proven ability to communicate effectively in English with all stakeholders.
