About the Role

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Highland Hills, Ohio. As a Home Health Physical Therapist, you will be responsible for providing high-quality rehabilitative care to adult and geriatric clients in their homes.

Responsibilities
  • Perform diagnostic tests and measurements to assess clients' mobility, strength, and balance.
  • Develop and implement individualized care plans to improve clients' functional abilities and independence.
  • Continuously assess and revise care plans to ensure optimal outcomes.
  • Educate clients and their families on rehabilitative care and activities to promote healthy living.
  • Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
Requirements
  • A current Ohio Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
