Job SummaryWe are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our hospice care team. As an LPN, you will play a vital role in providing direct patient care, collaborating with the interdisciplinary team, and supporting patients and their families during a challenging time.
Key Responsibilities- Provide direct, skilled nursing services to patients in accordance with their Plan of Care, under the direction of the RN Case Manager.
