As a Healthcare Liaison at Capital Health, you will play a vital role in connecting patients and families with essential resources and services. Your expertise will ensure seamless transitions throughout the care continuum, promoting optimal health outcomes.

About the Role:

  • This is a full-time position with an estimated salary of $72,000 per year, depending on experience.
  • The successful candidate will have a Bachelor's Degree in Nursing and at least two years of previous patient care experience as a nurse.
  • Experience with Microsoft Office and intermediate computer skills are required.

Key Responsibilities:

  1. Coordinate multidisciplinary case conferences, communicating with surgeons, medical oncologists, radiation oncologists, pathologists, and radiologists as needed.
  2. Maintain a database to track patient demographics and treatments, navigation activities, and quality and performance metrics.
  3. Act as an educational resource to staff on oncology care, staying up-to-date on new technologies, tests, procedures, and protocols.

Requirements:

  • Strong analytical and problem-solving skills.
  • Excellent written and verbal communication skills.
  • AHA BLS - Healthcare Provider and Registered Nurse - NJ licensure required.
