Community Intervention Liaison

Aunt Martha's Health and Wellness Inc. is seeking a highly skilled Community Intervention Liaison to join our team. As a key member of our organization, you will play a vital role in creating new connections and maintaining established relationships with community partners, while providing crisis intervention services to youth in need.
Key Responsibilities- Crisis Response Duties:
- Respond to crisis calls and provide immediate intervention services to youth and families in need.
- Develop and maintain relationships with community partners, including hospitals, schools, and law enforcement agencies.
- Provide day-time crisis intervention and case management services to youth and families on a no-decline basis.
- Complete all required intake and assessment documents, as well as, service plans and forms within required timeframes.
- Collaborate with Program Coordinators to obtain authorization for youth placements and ensure all required documentation is complete and accurate.
- Provide reunification, family-generated placement, or agency-arranged placement services, as well as, linkage and referral services specific to client and family needs.
- Enter participant information into eCornerstone and LAN-specific spreadsheets within 72 hours of crisis or intake.
- Community Outreach:
- Develop and maintain community relationships and a routine outreach schedule.
- Organize and attend community outreach events, such as National Night Out.
- Provide services to program clients during on-call shifts as deemed necessary by Program Coordinators.
- Provide life skills groups to responding areas and maintain an active case load as assigned.
- Monitor and supervise clients to ensure their safety and well-being during programming hours and transport.
- Bachelor's degree (BA/BS/BSW) required.
- Previous experience with at-risk youth preferred.
- Valid Illinois driver's license with a clear driving record.
- Documentation verifying current automobile insurance.
- Ability to work evenings and weekends, and be flexible in scheduling.
