RN Case Manager PRN

We are seeking a skilled Registered Nurse (RN) Case Manager to join our team at HCA Florida Orange Park Hospital. As a key member of our healthcare team, you will be responsible for coordinating patient care, managing length of stay, and ensuring appropriate resource management.
Key Responsibilities:- Coordinate patient care plans, manage length of stay, and ensure appropriate resource management.
- Facilitate patient progression and transition of care, identifying patients at risk for adverse outcomes.
- Perform comprehensive assessments of patients' psychosocial, medical, and discharge needs, as well as resource availability.
- Reassess patients' clinical conditions as indicated and ensure post-acute medical needs and level of care are appropriate.
- Make referrals to third-party payer, disease, and case management programs for recurring patients and those with chronic disease states.
- Act as a liaison through effective and professional communications between physicians, patients, hospital staff, and outside agencies, advocating for patients' rights, needs, and values.
- Graduate from an accredited school of nursing with an Associate's Degree in Nursing or higher (BSN preferred).
- Licensed as a Registered Nurse in the state of practice or hold an active Compact/Multi-State RN license.
- 1-3 years of clinical, hospital nursing experience required; 2 years of experience in Case Management or Critical Care preferred.
- Certification in Case Management or Utilization Review preferred; InterQual experience also preferred.
