We are seeking a highly skilled Registered Nurse to join our team as a Primary Care Clinic Nurse Specialist. This role will provide a wide range of clinical social work services in a primary care setting, including support and consultation to the care team in managing patients.

Job Description
The Primary Care Clinic Nurse Specialist will be responsible for evaluating psychosocial assets and deficits for the purpose of enabling patients to maintain the greatest level of independence. This will involve coordinating with others to establish a plan of care, providing short-term counseling, teaching, and brokering with other resources for caregiving needs.
Required Skills and Qualifications
• A minimum of one year of previous social work experience
• A Master's degree in social work (MSW)
• Licensed Social Worker Associate-Independent Clinical (Washington) or Licensed Marriage and Family Therapist (Washington) or Licensed Advanced Social Worker License (Washington) or Licensed Independent Clinical Social Worker (Washington) or Mental Health Counselor License (Washington)
• Demonstrated success as part of a multi-disciplinary team and commitment to diversity
• Care coordination experience in managed care systems, in-/outpatient settings, skilled nursing, or long-term care facilities
• Knowledge of social case management, conflict resolution, and team building practices
• Familiarity with Microsoft Office
