We are seeking a skilled Registered Nurse to join our team as a Home Care Specialist. The successful candidate will be responsible for delivering high-quality patient care in the comfort of patients' homes. As a Home Care Specialist, you will work closely with patients, families, and healthcare teams to develop and implement personalized care plans that meet the unique needs of each patient.
Key Responsibilities- Develop and implement comprehensive care plans that address the physical, emotional, and social needs of patients.
- Conduct thorough assessments of patients' conditions and develop plans to address their needs.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Provide education and support to patients and families to promote self-care and independence.
- Monitor patients' progress and adjust care plans as needed to ensure optimal outcomes.
- Current RN licensure in the state of employment.
- Minimum of six months of recent work experience as a home care nurse or equivalent experience in a related field.
- Graduate of an approved School of Nursing.
- Satisfactory references and completion of the Agency RN Qualifying Exam.
- Current CPR certification.
