At Linden Ponds by Erickson Senior Living, we are seeking a highly skilled Clinical Care Manager to oversee the care performance of our Assisted Living and Memory Care teams. As a key member of our leadership team, you will be responsible for providing clinical oversight and care to residents, ensuring their individualized service plans are maintained and updated to reflect their current needs.
Key Responsibilities:- Coordinate and provide clinical oversight and care to residents within Assisted Living and Memory Care, including medication management, pain management, wound care, nutrition, and respiratory care.
- Partner with the Assisted Living Manager to complete resident pre-screening/admission, re-admission, and ongoing assessments.
- Ensure resident's clinical assessments reflect their preferences, goals, and individualized clinical needs.
- Coordinate and monitor outside support services, such as hospice, therapy, and home health, following established plans of treatment.
- Track and trend clinical quality areas, including wound care, infection control, falls risk reduction, pain management, weight loss, and dehydration.
- A state-of-the-art community campus with ongoing expansion plans and design enhancements.

- A current RN license in the state in which you operate.
- A minimum of 5 years of experience as a registered nurse, with management experience in a Home Health, Assisted Living, and/or Memory Care licensed setting preferred.
- Experience and comfort working with an electronic medical record (EMR) preferred.
