We are seeking a highly skilled and compassionate Cardiology RN to join our team at OptumCare. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.
Key Responsibilities:
- Collaborate with cardiologists and other healthcare professionals to identify patients with high-risk chronic disease diagnoses, such as heart failure and ischemic heart disease.
- Conduct thorough assessments and develop personalized care plans to empower patients to manage their conditions.
- Provide education and support to patients and their families, utilizing motivational interviewing techniques and problem-solving strategies.
- Enter accurate and timely data into electronic medical records and other applications to ensure seamless communication and documentation.
- Participate in remote monitoring of patients, setting parameters and identifying changes in vital signs, and conducting ongoing discussions with providers to optimize disease management.
- Maintain current knowledge of heart failure, ischemic heart disease, and related treatments and medications.
- Establish trusting relationships with patients, caregivers, and healthcare staff to ensure comprehensive care and support.
- Conduct one-on-one visits with patients, triaging, educating, and assessing patient needs, and communicating these needs to the appropriate personnel.
- Solve problems by gathering and reviewing facts, selecting the best solution, and applying individual reasoning to devise or modify processes.
- Attend educational offerings to stay up-to-date on changes and comply with licensing requirements, ensuring all patient educational materials are current and maintaining knowledge of specialty and ancillary provider contract contents.
- Collaborate with the cardiology team to recommend policies, procedures, and standards affecting patient care for high-risk chronic disease diagnoses.
- Exhibit professionalism and courtesy with all patients, physicians, and coworkers.
Requirements:
- Current RN licensure and relevant experience in cardiology or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
- Ability to maintain confidentiality and handle sensitive patient information.
- Proficiency in electronic medical records and other relevant software applications.
