The ideal candidate will have a valid LPN license in the state of Colorado and a minimum of 1-2 years of nursing experience, preferably in home health or a similar setting.

Responsibilities will include:

  • Providing skilled nursing care to patients in their homes
  • Assessing patient needs, developing care plans, and implementing appropriate interventions
  • Maintaining accurate and thorough documentation of patient assessments, interventions, and outcomes
  • Communicating effectively with patients, families, team members, and other healthcare professionals to ensure coordinated care
